Community Mental Health Floating Support Service (CMHT Floating Support)

About

We provide housing related support in the community to Adults from the age of 18 years who have severe and enduring mental health issues and other complex needs.

We also provide intensive rehabilitative support at our Supported Housing Scheme, which has Staff on site.

Our support is geared towards promoting independence and assisting individuals to achieve their goals and aspirations at their pace via Person Centred approaches.

  • Maintaining / obtaining a tenancy
  • Tenancy sustainment
  • Support to apply for appropriate benefits and entitlements
  • Promotion of well-being and mental health
  • Budgeting
  • Development of a household routine
  • Signposting to relevant agencies (if required)
  • Homelessness prevention
  • Support to prevent admissions to Hospitals and undesirable moves into institutions.

    Referral via a Named Professional / GP / Housing Officer / Social Worker / CPN / Occupational Health / Permanence Team.

    Monday – Thursday 9:00am-5:00pm
    Friday – 9:00am- 4.30pm
